Clinical Depth That Matches Complex Support Needs
Some participants need more than a meal plan. Our dietitians regularly support:

Our online form takes a few minutes, and you don't need every detail on hand. If something's missing, we'll come to you directly rather than delay your participant's care.

What to Include in a Referral
The more of this you can give us up front, the faster we can move. Don't let a gap hold you back from referring:
- Participant's name, date of birth, and contact details (or their nominee's)
- NDIS plan management type (self-managed, plan-managed, or NDIA-managed) and relevant funding category
- Reason for referral and any relevant medical or mealtime history
- Current NDIS plan, if you have it on hand
- Anything urgent we should know before we call
Straightforward About Funding
We charge at the NDIS price limit, so there's no gap fee for NDIS or DSOA participants, and nothing for your participant to pay out of pocket. Every invoice carries the correct line item code, so nothing holds up your participant's plan.

Will I speak to a dietitian or an admin team?
Our admin team answers the main number, and they can set up a call with a dietitian straight away if you want to talk a case through before referring. Once your participant is linked with a dietitian, you have that dietitian's contact details and can go to them directly.
Does the participant need a doctor's referral?
No. You can refer directly as their support coordinator — we'll handle the rest.
Can you take participants outside Dandenong?
Yes. We offer home visits across Greater Melbourne and regional Victoria, a Dandenong clinic, and telehealth Australia-wide. If you're not sure your participant is in range, just ask.
Can you write a report for a plan review?
Yes. Our dietitians regularly write NDIS reports and letters of advocacy, linking recommendations back to your participant's goals. We advocate strongly for participants who need dietetic support.
